Assistance With Caring Service
The Commonwealth Home Support Program (CHSP) is a government subsidised program for Aging peoples over the age of 65 or 50 for Aboriginal and Torres Strait Islanders.
With Byron Shire Respite Service (BSRS), clients and carers receive the full benefits of a personcentred Care Management model.
This includes an assessment, updating of care notes, updating/ review of Care Plans, allocation of client specific staff, co-designing Care Plans and communicating strategies based on changing needs, and advice given by the Care Management team.
This model is the bedrock of our service, equips our staff to be well informed to each client’s needs and enables the organisation to provide in-depth person-centred care
Without this model in place Byron Respite would not be able to provide the full extent of care provision and may place a client or staff member at risk.
If you are transitioning from the Commonwealth Home Support Program (CHSP) onto a
self-managed Home Care Package, BSRS will continue to provide this Care Management model for you or loved one, but the model is no longer subsidised. In order receive same level of Care Management service that you did under CHSP, BSRS has in place a small annual fee (for HCP levels 3 & 4 only) enabling our organisation to continue to provide the level of care you are accustomed to.
