dementia care

Our person-centred service starts with a detailed assessment to gather information about your history, favourite activities and pastimes and interests. This assists our Care team to provide holistic care for you and your family, and this enables us to monitor changes and development of complex needs, so we can put new strategies in place to maintain and support independent living.

You can expect support from our highly qualified Support workers that can be provided in a variety of settings including the home, community or at our day programs. One on-one structured activities include diversional therapies including gentle exercise, arts & crafts, music and cognitive stimulation and enable people to experience, develop and social interaction while offering respite to the carer.

We understand that the person with short term memory loss or dementia, leads a life of moments. We aim to create wonderful moments that put a smile on people’s faces, a twinkle in their eye and most importantly trigger memories. The feelings evoked by this will linger after the memory has left.

When can service be provided?

A Welcomed Break is provided under the Commonwealth Government’s Flexible Respite and Centre-based Respite Programs that aims to be flexible and responsive to the concerns of the carer and the person for whom they are providing care. This service can work in partnership with other services to provide the best possible care options.

The program operates every weekday and you can participate under the government subsidised Commonwealth Home Support Program (CHSP) or use your Home Care Package funds.
